Enliven Individualised Funding offers support for people receiving this government funded service. Our services includes providing you, your family and whānau with information and guidance to let you manage the home and community support services you receive. You get to choose who provides your support, how it is delivered and when.
As this service is capped, we operate a waiting list via the Southern District Health Board Clinical Needs Assessors.
